Each statement is rated on a 1–5 scale based on how consistently the behavior shows up within your team.

4 – Mostly True
This behavior occurs frequently but may not be fully consistent.
3 – Sometimes True
This behavior appears occasionally but is inconsistent.
2 – Rarely True
