How the Scoring Works
 

Each statement is rated on a 1–5 scale based on how consistently the behavior shows up within your team.


5 – Consistently True
This behavior is clearly embedded in how the team operates.

 

4 – Mostly True
This behavior occurs frequently but may not be fully consistent.

 

3 – Sometimes True

This behavior appears occasionally but is inconsistent.
 

2 – Rarely True

This behavior happens infrequently.
 

1 – Not True

This behavior is largely absent from the team.
